Delay in issue of voter id card

I have been a resident of Kolkata since birth. Though my details and photo were taken twice in the last two years i have never received my voter id card as of yet. Last time though i was allowed 2 vote i still havent received my voters id card.
I will be grateful to you i[censored] could please look into the matter on an urgent basis.

Past 10 years i am staying in bangalore, still now i didnt get voter id, In 2007 i went to our ECO and submited voter id form and they given recepit after that i went to that office i asked then to give voter id they told old office is shifted now your xerox copies not avail they told again i submitted in 2009 that is 18th Jul-09 they given recepit also still with me they told that we will inform when you have come and collect voter id, but still now they didnt call me and no intemation also. this time i didnt vote also. please up me.

After completion of age 21 years I have submitted form 6 will all detail informations, documents & Photo twice in the last two years. Again in April/2010, during the campaign of Election Commission again I have submitted form 6 with photo at my nearest Booth No. 7(Ram Mohan Roy Seminary School, Patna to the staff deputed by state Govt. But still my neither my name was en roll in Voter List nor I have never received my voter id card as of yet.
