Employee's Providents Fund Organisation [EPFO] — Epf withdrawal of account no dl/23859/014409

Please help me withdraw my epf amount from dl/23859/014409 account. I have tried several times to withdraw/transfer but nothing seems to work

Some background
- the employer has shut down
- my bank account is also closed

When the employer was available, i had requested for withdrawal and the cheque had also come to the bank, but was sent back due to name mismatch. After that the employer shut down and i have no where to go.

I am based in pune, so i visited the epf office in pune (Swargate), but they didnt help me.
