Claim id and acknowledgement number are required to track your status of your reiumbersment claim.

Is serial number or acknowledge number is same as our claim id?

Dear sir,

Our factory is situated in murbad, thane district, we are not getting the proper treatment here in esic since long time, many of our workers complaints to us regarding the poor treatment,

We would like to bring to your notice one of the recent case of our employee mr. Chandrakant ghude as below,
His esic insurance no. Is [protected]

There was a pain in his shoulders and not able to move up his hands, he went to many esic hospitals for the treatment but there was no improvement in his health,

Finally, he went to a private hospital and got better treatment please find below his case history in details.

1 - 1st visit to esic hospital at murbad on 08/10/2019 for the treatment
2 - he visited again for the same problem on 02/11/2019
He took the treatment for one month but there was no recovery.

3 - murbad esic sent him to mulund esic for the further treatment on 02/12/2019, he took the treatment for 15 days from mulund esic but there was no improvement in his health.

4 - then on 13/12/2019 mulund esic told him to go to kem or jj hospital for the further treatment,

5 - he went to kem hospital on 14th dec-2019 but he could not visit to doctor because the dr. Was not available on that day.

6 - on 16th dec-2019 our employee mr. Chandrakant ghude decided to go to the private hospital and take the treatment because he was suffering from the problem and was not getting proper treatment.
He went to shree hospital at kalyan and started treatment there and within 15 days he was feeling better.

7 - after 15 days of treatment he joined the job again and dr. Adviced him to take the treatment for three months. He spent around rs.10, 000 to private hospital.

8 - 03.01.2020 he went to ulhasnagar esic to claim the expenses for which he spent at private hospital but they refused to take the claim and also was not ready to give the medicine for next three months as per the treatment he got from the private hospital.

So we hereby request you to kindly do the needful in this regards so that mr. Chandrakant ghude can get the money back which he spent in a private hospital and need some action against the esic that if the treatment is not available with esic then they should reffer the patient to private hospital and not to kem or jj hospital.

My name is premalatha, ESIC NO: [protected], I delivered a baby on 21st November 2019 at Chetan Hospital, Puttur. But the baby died on the same day as intra uterin death. One month maternity leave is already provided by ESI but to get other 5 month leave, My ESI doctor told me to meet him in one month after the delivery date. I have visited ESI hospital more than 10 times in December month but most of the time he was not available. Today we met him in the hospital and he is asking the original discharge summary report to give maternity leave. Actually I have claimed delivery charges through my husband insurance. Hence the original discharge summary report is with my husband insurance company that is United health group. They will not give the original report once the claim is settled. So we put doctor seal and signature on the Xerox copy but the ESI doctor Nithyanandh is not accepting it. Kindly help me to get maternity leave and salary for this period. Thanks for the help.

I am working in HGS from October 6th 2014 to till now and every month ESIC contribution was paid til October 31st 2018. It was very good journey until this bad day August 26th 2018 in my life. I was suffering from more fever and back pain hence I visited the ESIC hospital Rajajinagar. Doctors done many blood tests and finally Diagnosed that It was a 'Early T Precursor Acute lymphoblastic leukemia (T-cell ALL or Blood cancer) - high risk. Doctors advise me to admit immediately for the Chemotherapy treatments.
As I am ESIC Contributor I was eligible for ESIC medical benefits and Sickness benefits my treatment done under the same. I was concerned respective ESIC medical referee and he approved the Extended Sickness Benefit (ESB), without contribution to tthe ESIC, so I can eligible treatment to until August 31st 2020 as per approved ESB benefit.

For the purpose of paying ESIC salary and ESB benefit from ESIC . Now ESIC- Bommasandra branch manager was asking for the contribution payslip from October 2017 to March 2019. As per their needs I submitted Wage particulars request form to my respective ER and I received Wage particulars and submitted to ESIC bommasandra branch manager, branch manager not accepted the wage particulars due to it’s showing it’s exceeded rupees 21, 000.

In office every month i was doing extra hours work, for the extra hours company paid additional incentives with the salary, for this reason my salary exceeded the rupees 21, 000, it’s not exceeded the ESIC wage limit.

Branch Manager denied to approve the ESB facility from last 15months due to this I did not received any medical benefits from ESIC.

Even lot of follow-ups, requests, clarifications about wage particulars Branch Manager is not ready to listen my problem and he is not concerned about the seriousness of my health issue as I am suffering from Critical illness(Blood Cancer). I can’t travel daily due to this my health is not improving it causes physical and mental illness requesting to solve the issue at earliest.

I was suffering physically and mentally from this health condition and I spent more than five lakh rupees for my treatment apart from ESIC medical services, currently I need minimum six to seven thousand rupees for monthly blood tests and cancer medication. I didn't have any other source of income, my family also depended on my earnings and they spent more money for my treatment and financial crises become high now.

With due respect i want to state that i am sanjeev arora s/o sh. Mohan lal having esi no. [protected] under employee of polyplastics industries (India) pvt. Ltd., yamunangar haryana.
My father sh, mohan lal suffering from heart problem than esi hospital yamunanagra referred the case to pgi, chandigarh on dated 25.10.2017 having cr no.[protected] after discharge from pgi chandigarh on 27/10/2017 we are submitted the expenses with all documents to esi yamunanagar on dated 08/11/2017 after that esi yamunanagar raise some query (I. E counter sign missing) we are solve these query & again submitted the documents on dated 09/02/2018.
Now i am chasing for my refund for rs. 1, 03, 371/- but our esi yamunanagar said your document sent to karnal office on dated 09.04.2018 through dispatch no. 224 & payment also released by the karnal office & if you have any query so please visit to karnal office & follow up for refund.
We have received the reply letter from esi jagadhri (Letter enclosed) ref. No. [protected]/[protected]/309/23/07/2018 you are not eligible for super specialty treatment so that your refund is not entertained but your medical expenses relates to govt. Organization so document send to director health services chandigarh through civil surgeon karnal on dated 19.07.2018 & we are also chasing to civil surgeon karnal on dated 08.01.2019 through letter through on dot courier having docket no. [protected] but no response received till yet

Secondly i am member of esi for long time & due to the change of my job my esi no. Also changed time to time as detail mentioned below so that i am eligible for super specialty treatment

It’s my humble request to you kindly clear my expenses bill as soon as earliest & i am so tensed and more than two year my document and me move one table to another table & still my hand is blank.

I am poor person & taking credit/loan against this amount & i am pay the interest on this amount. Father is also retired from pvt. Job & i am single hand earning.

Now its time turns to 8.37.. Its very poor to look this dispensary because there is no more staffs are present at this time.. There is no punching machine i found. Peon told me that dr is leave today.. Workers like us are coming here with half day leave.. Such time there is no dr in this dispensary, what we do???... Please provide more staffs and give them punctuation on their duty. Thnks

सर मैंने अपने पिता जी का इलाज बिना किसी पूर्व सूचना के राज्य से बाहर करा लिया हे 2017 मे तब से आज तक पेमेंट के लिए हमे परेसान किया जा रहा हे हम जब भी इटावा अस्पताल मे फ़ाइल लेकर जाते ह तो बो एक नयी कमी निकाल देते ह अभी अंतिम बार जब हम अस्पताल गए तो डॉ साहब ने मना केआर दिया की अब कभी नही पास होगी फ़ाइल सर 2 साल से परेसान हो रहा हु कानपुर इटावा दौड़ रहा हु कोई मदद नही हो रही ह मदद करने की कृपा करे

Dear Sir/ Madam,

We are been following up with ESIC from 22nd October 2019. Our employee Mr. Rahul Kumar Rajgirish Pandey ([protected]) wish have D 1/3 Kalol, Ahm., GJ (ESIS Disp.) dispensary for his family. We sent mail to the IT department of ESIC in Delhi. They told to meet the caseworker.
In Gujarat we met the caseworker, they told us to get it corrected form Mumbai since the mail code is in Mumbai and we are paying from Mumbai only.
When we approached the EDP department, they also sent mail to IT but still, we have not received any help.
Request you to please help since we are not getting any support from ESIC department

Sub: grievance filing with regard to employee state insurance benefits.
I am writing to bring to your notice the difficulty my wife and i have been facing while claiming the benefits of employee state insurance provided by the employee state insurance corporation (Esic). We live in tenali town of guntur district, andhra pradesh and both o[censored]s work as teachers at a local private school. Both o[censored]s are diabetic and visitmanasa clinic run by dr. Sharath kumar regularly. For the last three months, we have been denied medicines for diabetes and the reason being told is that the government has not been sending supplies to the center. Last week when we visited the center and requested for medicines we were told that only iron supplements were available and we can take the same if needed. Such apathy being shown to diabetic patients by either providing them with nothing or mere iron supplements is a cause to worry. It has become very difficult for us financially to buy medicines from private pharmacies and the issue is taking a heavy toll on our health and finances.
I request you to resolve the issue o[censored]navailability of medicines in the concerned center and relieve the stress and difficulty of many such other patients, at the earliest.

I am at esic hospital rohini delhi - 110089, we all (Patient) have facing inconvience, because of the registration counter isn't open till now, it's 8:50am and the timing for opening is 8:30am, even the security guard also saying there is no one who listen yours, don't loss your energy.

This is the incident with registration counter no 2 most probably this is the issue with all the other counter also.

How could be a country will grow, if the people doesn't get the proper treatment.

Please this is our humble request for you guys take a strict action who is responsible for it.

03/11/2019 is din me mere maa ko sath hospital le gaya tha lekin jab me waha pe gayaa to muze doctor ne documentory formalities puri karne ke liye kaha o mene puri kee lekin jab me unake pass gaya to pehelese hi report banakar rakha tha usme mera khali sugar ka ek hi test kiya aur me us doctor ne galat report diya, aur dawai ke le mana kiyaaur dusare din wapas bulaya dusare din bhi pura din bhar bithake rakha lekin dawai nahi di aur puchane gaye to ulta sidha answer diya tum bahar se dawai kharid lo hamare pass dawa nahi milegi aisa bhi kaha aur dhamaki di kisiko batana hai usko batao me kisise darta nahi mera koi kuch bigad nahi sakta.

Esi panel doctor office at picnic garden open at 11 am and closed at 1.30pm even the actual office time is 10am to 5pm. I complaint against this but no any action taken yet. This worst system of government services harasses many people. My mother is suffering from pain of knee and she is not able to move his both legs i went to esi sealdah in emergency, doctor give pain killer injection and advice us that bring form no.3 and visit outdoor with concerned doctor when i went to esi panel doctor picnic garden on monday at 2pm but no one was there even office staff. I request to please raise this issue to resolve the matter immediately.

I am to say that i have been employed for nsso jaipur by g. A. Digital web world pvt. Ltd. No.1, hargobind enclave, vikas marg extn. New delhi. From 20th march 2019.in this regard my esi deduction was made by the company every month but it has not shown on esic portal and due to this the benefit of treatment could not be taken in esi hospital. मैं पिछले 5 दिन से esic डिस्पेन्सरी जवाहर कॉलोनी फ़रीदाबाद मैं चक्कर लगा रहा हूँ यहाँ पर हॉस्पिटल की पर्ची बनाने वाला स्टाफ बहुत ही लापरवाह और गैर जिम्मेदार है यहाँ पर सुबह 7 बजे से मरीजों की लाइन लगनी शुरू हो जाती है और स्टाफ 8 बजे के बाद ही आता है और आने के बात गप्पे मारते रहते हैं बाहर मरीजों को बैठने के लिए भी जगह नहीं मिलती और बुजुर्ग महिलाएं और बूढ़े गंदगी मैं सड़क पर व सीडियों बैठने के लिए मजबूर रहते हैं। रेफर पर्ची के बनाने के लिए भी बड़ी लंबी लाइन लगानी पड़ती है। किसी को डॉक्टर को दिखाना हो तो बाहर बैठा असिस्टेंट व पर्ची बनाने वाले सभी से चाहे बूढ़े, महिलाएं हों या फिर कोई और सभी से बड़ी गुस्से और बत्तमीजी से बात करते है और गुस्से मैं esic कार्ड भीं फेंक कर मारते हैं । दवाई देने वाला कर्मचारी दवाई समय पर नहीं देता और घंटो लाइन लगाने के बाद भी दवाई पूरी नहीं देता। अगर दवाई हॉस्पिटल की जगह दवाई डिस्पेन्सरी से मिलने पर भी दवाई नहीं देते और घंटों लाइन मैं लगा कर पूरा नंबर और कार्यवाही करने के बाद मना कर देते हैं की दवाई बाहर से खरीदो । मैं बड़ी मुश्किल से 5 दिन चक्कर लगाने के बाद हॉस्पिटल की रेफर पर्ची बनवा पाया हूँ और यहाँ के कर्मचारी बहुत की लापरवाह और बहुत बेकार हैं। कृपया करके जवाहर कॉलोनी डिस्पेन्सरी के कर्मचारी को हिदायत दी जाए की अपना काम सहीं से करें

There is no facilities of esic hospital in our town. Due to this neither proper information is available nor approve claim. We have requested to esic commissioner j. P. Sharma and m. A. Khan indore for visit in our institute but there is no action towards it. There is a esic hospital in sanawad but it is situated 60km away. About this situation we have also informed to commissioner but till date there is no action taken and no body visited our institute.
Therefore i request you to take necessary action for complain information about esic benefits, claim settlement.
I request you kindly look in the matter and doing the needful.

Dear sir/madam, main aaj maa ko esic hospital main lekar gaya. Vah jaane ke baad nurse bole ki app documentary formalities puri karo, main ne formalities puri kee aur doctor ko dikhai doctor bole patients ko dekha aur bole aciditi ka problem hain doctor ne muze ek slip dee usame likha tha kee apki pura test huvaa hai, maine doctor ko bola apane to check kiya nahi, doctor bole aapko jise batana hai batao main nahi darrta.

Hi,
My name is raushan. My mother is admit in esic basai dara delhi since 5 days. Her condition is being critical day by day. When doctor comes on round gaurds ask to attendent wait out side even my peteint is not able to talk. I ask to gaurd she said "ja jisko bolna h bol nhi jane de rhi". Then other nurse also came and said close the door nd no one will come. Please look into this behavior because people like me are feeling like guilty to come hospital. It's been 5 days i even don't know what happen to my mother because i am not allowed to talk to doctor.
