Golden Forests India Ltd — claim form error - account no/name / amount / receipt no. mismatch

I am filling claim form of golden forests india ltd. 10/10/2018
After filling all my details and submitting the details in the first section, when i click next, a pop-up message shows "name/amount/receipt number mismatch" and sometimes "account number is invalid". I have entered correcely account no.

Reciept no. Ls/97/169260

Please suggest what should i fill in reciept no. And account no.
Someone telling pre-fix this n that, i tried lots of options. Please help, what should i fill in reciept no. & account no.
