Sir,
My self laxmi bisht, i would like to inform you that i have submit online claim form for golden forest india limited rd a/c no. Rud97839, receipt no. Ru/99/16756 on 17 july, 2019.
I have not receive the cheque till today.
Photo of claim form are attached with this letter and my new address are given below — ‐-

W/o shri rajendra singh bisht,
259/2 vijay colony, hathibarkala,
Dehradun - 248001.
Hence i request to you that kindly take necessary action in this regard.
