HCL Technologies — annexure-k form

I have raised a request to transfer my pf from hcl to ntt data and request has been completed, but the hcl pf amount is not showing there, so i contacted pf office they told me need to submit annexure k form from hcl to pf office, but concern person from hcl is not responding accordingly.
