Cash not dispensed from ICICI ATM

I have an account with HDFC Bank,
on 8th OCT 2008, I tried to use HDFC Visa card in an ICICI
tried to withdraw amount Rs 15000, amount was debited from my account but cash was not dispenced from ATM :

Logged a complain with HDFC on 9th OCT :

Upon the request of HDFC customer care executive on 13 OCT, submitted a written Charge Back form on 14 OCT 2008.

Non disbursement of amount by ATM
