ICICI Prudential Life Insurance — revival of closed policy

My policy number 15345376 has been closed due to non payment of premium. I took the policy in the year 2011 and also paid the premium in the year 2012. The agent said that now the policy's has been closed so neither revival nor payment of premium is possible. I could not pay the premium as my son suffered from septicemia and is now suffering from cerebral palsy. Either the revival be allowed or my money of two premium be refunded.

I have taken policy from iciciprulife 7 years back. now that baond was lost. idont know what is the policy number.How i will get my policy bond number
