Indian Bank — issue at my upi transaction

I am Shri Ananda Sankar Behera bearing Acc. no.[protected] in Baramunda Branch Bhubaneswar, Odisha.
its Happened twice with me when i use UPI transaction it holds as always, where as Money Deducted from my Account and not credited to the sender's account.
CASE1-Amount of 2319/- Not credited to CAPITAL FAST dated on 3rd.April.2019
CASE2-Amount of 1360- Not credited to PAYTM WALLET dated on 9TH.April.2019
Nither Credited to Sender's acc not returned to my account.
