Indian Overseas Bank — atm debit card locked in hdfc atm - request to block the atm card

I shankar siddharth is having iob sb a/c no.[protected] under the branch sivaganga. My iob atm card no.6521 xxxx xxxx 2661. On 23.12.2017, i tried to get money from hdfc atm, k. K. Nagar, chennai. I have given request for a atm card before 2 weeks
Still i didnt receive my atm card
also the bank assistant manager is not taking any responsibilities about customer complaints
Please take any action against her and provide me atm card
