Madam/Sir,
I Mrs. Rashmita Mohapatra on Jan 02, 2017, tried to pay Rs.29, 023.00 through my debit card no. xxxx xxxx xxxx 2418 of SB A.c. No.[protected] of IOB Chandrasekharpur, Bhubaneswar, Odisha. As the transaction was failed on 02.01.2017 at around 4.00 pm, I enquired about refund at Bhatrat Motors, Cuttack Road, Bhubaneswar. They confirmed the failure of transaction. Till date the amount not credited back to my above SB account.
