Indian Railway Catering & Tourism Corporation [IRCTC] — money deducted but booking failed

Dear sir,
While booking a tatkal ticket on 11 oct 2018 from ssm to hwh transaction was successful but booking failed twice and hence booking not done but money deducted from my account. Waited in the hope that it will be credited reversed automatically but not yet done.

Today I booked an IRTC ticket using the IRCTC official app.
When I booked a ticket, all the processing was completed, but in the end, my amount was withdrawn from my bank account, but my ticket was not booked.

I travelled in 12220 from Hyderabad to Mumbai on 12th with one no: [protected] in 3rd ac. Cleanliness is horrible. Rude staff. Very reluctant to speak and solve the problem. Never expected this type of hospitality especially in this train. This is second time for me . Once when I travelled from banglore to Secunderabad by Rajadhani Express.
