Lic — Complaint Title / Subject: Request for Deactivation of Agent Code and PAN Removal from IRDAI Portal

Respected Sir/Madam,

I, DHARAM RAJ (PAN: AWSPR0552J), was appointed as an insurance agent with Life Insurance Corporation of India (LIC) bearing Agency Code LIC07389394.

I formally submitted my resignation from LIC and my agency was ceased on 01/08/2019. I have a copy of the cessation certificate issued by LIC. However, my PAN number is still active ("Inforce") on the IRDAI Portal, which is creating a significant problem for me.

I have recently received a job offer from another organization, but I am unable to join because my PAN is still active in the IRDAI system . I have already contacted LIC regarding this issue, but they have not been able to resolve it . My registered mobile number with LIC is also no longer active, so I am unable to use the ENVOY Self Deactivation portal.

Therefore, I humbly request you to kindly deactivate my agent code and remove my PAN Number (AWSPR0552J) from the IRDAI Portal at the earliest so I can join my new job. I am attaching the following documents for your reference:

1. Copy of Cessation Certificate issued by LIC
2. Copy of PAN Card
3. Copy of Aadhaar Card (with active mobile number)
