I am retired officer-staff of central bank of india. Today i have submitted all necessary particulars on line in the prescribed format for renewal of health insurance policy (Option ii with domiciliary treatment) and authorised the bank to debit my savings bank account towards insurance premium rs.36, 998/-. I do not receive confirmation otp to my registered mobile number, though display appeared on the screen as below:
"mdindia on line.com says: information updated successfully".

I require your confirmation to my email address "[protected]@yahoo.co.in", stating that the policy will be renewed for the period 1.11.2017 to 31.10.2018, recovering premium to the debit of my savings bank account with central bank of india, tallakulam branch (Chennai zone, madurai region). Ifsc code of the branch is cbin0280915.
