with due honor i am very sorry to inform you that in spite of several representations
To approve and sanction of rs.18000.00 for operation of cataract at disha eye hostital
Fixed on 22.02.2017 but it is intimated that disha has not yet submitted all the requisite
Documents as per your requirements.

I have submitted all the requisite documents including biometric report and every thing
To disha tpa’s office to the man concerned madam hena.

It is not understood why disha has not yet submitted duly filled annexure c as per
Agreed ppn package,
please expedite disha to receive the same as per your requirement, so that customers are
Not harassed due to the negative motivation of the disha.

Please ensure that my operation is tabled on 22.02.2017 as per scheduled .
