Dear Sir/Madam,

I am writing to request the issuance of my Consolidated Marksheet for the Bachelor of Computer Applications (BCA) program, which I completed in 2015 from Mahatma Gandhi University, Meghalaya.

I have received all six semester marksheets and the convocation certificate. However, I have not yet received the consolidated marksheet, which is required for my admission into an MCA program.
