Mphasis — Provident fund claim

Am not able to withdraw my pf.It has been sent back the company.
My mobile number has been changed and I cant find my details.
The company messaged me to do it online.
I need help on this trying since 2 months.
The form came back and when I dont know my password as well. I cant even view my passbook of pf.
I need all my details of pf.My employ ID was 2286452.
