Mphasis — Unable to access PF passbook on EPF portal and PF withdrawal

I have a query about EPF.
When I login to my account and try to see my EPF Passbook it does not show any records, it shows the message as Passbook is not available to this member ID as this pertains to exempted establishment(i.e trust).
Please advise what should I do. I tried to raise ticket from the link for Ex mphasisn..https://mphasisin.service-now.com/ExEmployeeServiceForm but after filling all form it is showing submit button disable.
Please help me what should I do..and whom to contact
