My name is sandhya krishna kishor tripathi I have baroda health policy no. 301800/48/15/[protected] I sent dd on dated 24.06.2016 for renewal of baroda health policy at laldarwaja, ahmedabad branch and also informed by mail. But till date i haven't received my mediclaim policy i also enquire in bank personaly two or three time but they told me that no any policy received. I have received a cheque no 264418 dated 30.09.2010. against settlement of my m/cycle claim no.320302/31/10/62/9000021 srl.no ; 1 mp-39-b-1431. above mentioned cheque send to national insurance co. shajapur br for re validation .by regd.post recd in br on 18.09.2015. i have not received re validate cheque till date. please help me .
