I believe you are the point of contact for national insurance company ltd. To brief you my name is ranjana and work at ernst & young company bangalore.

National insurance company is the insurer for ernst & young and the third party was united health care upto march 2017. For this year 2017 i have 2 reimbursement claims policy number-602200/46/16/[protected] which was for my dad as he was diagnosed for cirrhosis of liver due to hepatitis b at st. Philomena’s hospital bangalore unfortunately he passed away in june 2017. The problem with the first claim id[protected] &[protected] for january 2017 (17th -19th jan 2017) is that uhc is expecting a document “treatment chart” and the hospital says that they don’t have any separate document called treatment chart instead hospital has indoor case papers and nursing sheet and asking uhc to refer to that which i have already submitted but uhc has denied my claim for this purpose.

The second claim id[protected]th – 19th march 2017) the problem is a temporary treating doctor (Dr. Ranjith shetty) had done the distension of abdomen fluid for my dad and he has sent the patient history as alcoholic which is incorrect. Later this doctor (Dr. Ranjith shetty) has also written a letter saying the patient is non-alcoholic attached his letter written in march for your reference. This temporary doctor was unaware of the fact that the patient was a hepatitis b positive due to which it led to cirrhosis of liver and not because of alcohol. The doctor who had provided the letter stating the patient is non-alcoholic is the actual treating doctor (Dr. Praveen) who had treated my dad from the start i. E january till my dad’s death certificate was provided. Only the march treatment was done by dr. Ranjith shetty which has caused a chaos to this claim. Dr. Praveen the actual treating doctor has also written a final letter stating the cause for cirrhosis of liver is due to hepatitis b (See attached)

Note that i have also reached out to the grievance cell o[censored]hc but no response from their end.

Incase this has to be forwarded to any other relevant contact please do the needful as i want to approach the grievance cell of national insurance company to take this forward as there seems no end to this case. I need justice to this.

Sir, I have lodged complaint online grievance portal vide No. GRI[protected] but till date I could not find the status of payment of my claim. Claim was approved on[protected] but till date I do not receive the payment. Is it a prompt service of your company.

It seems you are doing nothing for the customer but only come to the office for cup of tea and gossips. Please you should read or take care of your own commitments.
