Sir/madam, It is intimated to you that, on 20.02.2017 i had meet an accident for which i had been admitted to basu’s clinic, hakimpara, siliguri under dr. Abhishek agarwal, mbbs and on 22.02.2017 i had been operated on my right shoulder. The requisite documents had been submitted by me to national insurance co. Ltd, Branch-i, hill cart road, siliguri Received copy enclosed On dated 18/04/2017 i had been received a letter from md india health insurance tpa private limited regarding submission of additional investing reports supporting the diagnosis – x ray shoulder/ ct shoulder – pre operative & post operative as well as mlc from hospital, which i had submitted in original. Ltd, Branch-i, hill cart road, siliguri Received copy enclosed Till date my claim has been not settled which was amounting to rs. 93, 917/- (Rs. Ninety three thousand nine hundred and seventeen) only, and no further information has been received from insurance company till date. However i have communicated with their officials as well as to customer care several times, but no positive response has been received from their end. During my follow up for the claim by personal visit as well as telephonically they had assured me several times that claim will be settled by the end of this month and same commitment remains for the next month also, but claim remains unsettled. Further i had communicated to the officers of irda shri chittaranjan sarkar (Copy enclosed) and shri a. K. Patnaik (Copy enclosed). Further i had also communicated to shri tarak nath, officer of md india health insurance tpa private limited by mobile no. [protected] & e-mail id [protected]@mdindia.com, but yet no response has been received. On 18/10/2017 when i visited to national insurance co. Ltd, hill cart road, siliguri, branch – i, to enquiry about my claim, there they have given the draft repudiation statement in which they states that they have not received any additional documents (Copy enclosed), whereas i have already submitted all supporting additional documents in original to national insurance co. Ltd, hill cart road, siliguri, branch – i, on the date mentioned above and i cannot obtain x - ray & ct scan plats again once it is already submitted with proper received copy obtained from the branch. So, i cannot understand how they are saying that till date they have not received the documents, and i don’t found any fault from my side as i have already submitted all documents. A letter addressing to grievance redressal officer, national insurance co. Ltd, grievance redressal officer – 3, middle town street kolkata – 700071. (Copy enclosed). Now with much hope and getting fair response from your end, i am requesting you to kindly throw light into my case as it is already too much late and do favour to me.
