National Insurance Company — Non receipt of Mediclaim Insurance Policy

I got my above policy renewed from your office on 3rd. June 2015 and paid an amount of Rs.10125/- through cheque No. 3309809 dated 2nd. June 2015 drawn on union Bank Of India NIT Faridabad and the same has been encashed. But it is a matter of concern that till date I have not received the Policy Document.
