National Payments Corporation of India [NPCI] — failed transaction and debit of money for same

Following failed transaction, money is debited from bank account neither received by recipient nor reversed to debited account.

Further I sought reply from Bhim app, It mentioned that account was not debited at all despite having debit message with me. I had mailed same to support upi & banking ombudsman of Mumbai region but got no response eighter.
