Pacl India — pacl certificate lost what is the solution.

Dear sir my pacl certificate was lost and now i want to apply for refund. Is ani solution. My certificate no. As per receipt u[protected] named neelam devi w/o sh. Suresh parsad. Without this certificate no claim will be allowed.
You are requested please solve the above said problem. If there is any process to find claim please tell me. I humbly thankful for this.

I have certificate of pacl india ltd on my wife name in the cerficate the surname is vaghani but in pan card and all my wife document the surname is patel so i request you can i have upload the document
