lost pan card while traveling-- so would plz kindly tell the procedure to reissue Pan card--

As per status my pan card was dispatched by postal on 13.12.08 (PAN NO: CCNPS1350L) but not yet received,

i have lost pan card a month ago. what is the procedure to get a new one. should i apply for a new one or can i get a duplicate.

I had applied for a PAN which I was issued as CBZPS6118P. But till date I have not received the Card. When verified with the concerned dept. I was told that the same has already been couriered to my address on 29.11.2008 from Kolkata. Then where did it go other then me?

Pls. help me getting the same or issue a new card and dispatch.
