Passport Office Surat — Urgent: Complaint regarding unauthorized name modification (Nirali to Niraliben) - File No: SU[protected]

I am writing to lodge a formal complaint regarding a clerical error in my passport application (File No: SU[protected].

Despite providing my School Leaving Certificate, PAN Card, and Marriage Certificate, which clearly state my name as NIRALI, the current status of my application indicates that the name is being processed as “NIRALIBEN.”

Key Points of Grievance:

Unauthorized Modification: I explicitly requested the addition of my parents’ surname as per my legal documents. The suffix “BEN” was added without my consent or supporting documentation.

Document Discrepancy: My primary identity documents (School Leaving Certificate) do not contain the suffix “BEN.” Printing this will cause significant legal and travel issues in the future.

Failure of Grievance Redressal: I have contacted the National Helpline and the RPO Surat office[protected]. I was informed that I cannot book a new appointment while the file is "In Process, " yet the office is proceeding to print the passport with the incorrect name despite my notification of the error.

Requested Action:
I request the RPO Surat to immediately halt the printing/dispatch of the current booklet and rectify the name to NIRALI KALPESH DOLAR as per my submitted documents. If the passport has already been printed, I request a Gratis (Free of Cost) Re-issue, as this is a clerical error made by the department.
