Police Clearance Certificate — printed application form not accepted at local police station

I did online application for pcc requiring gate pass formalities at indian navy and ongc.

The application in printed form downloaded from online system after filling all necessary columns and payment, not accepted by local police station (Nhava-sheva police station).

While discussion with officer at nhava sheva police station understand that my application form should have following information
1.0 date/month/year of current residence
2.0 yes or no at the place where stated as do you need to verify your previous address.

For your kind information both above has been mentioned in online form but could not seen in printed form. I shown my mentioned point with online form but officer in the police station denied to accept it and asked me to correct it.

I tried to edit but not possible now.
Please let me know
1.0 printed form of application is wrong or right
2.0 if wrong then how to edit it
3.0 if edit not possible then what next to obtain pcc
