1. Kindly make a reference to my proposal no s 23461/23 ea/40 dated 18 oct 2003 as confirmed vide your latter dated 17 nov 2003 (Copy inclosed).
2. I have retired from the army on 31 may 2016. The recovery of premium to the pli was made by pao (Or) sikh li fatehgarh (Up) till 30 apr 2016.
3. After retirement i was unable to pay the monthly premium as i was not issued any pli policy by your office. I contacted the office of apo chandigarh who expressed their inability to accept the premium in the absence of pli policy.
4. I therefore humbly request your good self to kindly forward my pli policy on the address mentioned below. I shall feel highly obliged for them act of kindness.
