Dear sir,
I beg to draw your kind attention to the conditions of my locality. The sweeper and sweeper supervisor are very careless drains remains choked. They give out foul smell and mosquitoes. It is very harmful for helth. And the sweeper never comes to clean this.Therefore i request you to please take appropriate steps against this serious problem.

I hope you will be kind enough to look into the matter and get the place cleaned up as early as possible.
