Sharekhan — FATCA form - signature mismatch

Dear Sir,

Please register this as complaint.

I am a NRI, holding demat account with Sharekhan from 2009

Name : N.SUDHAKAR BABU
Demat ID : IN300513
Customer ID : W75913

My account is under hold for submission of FATCA Form.

From last one and half year, I submitted more than 10 FATCA forms, but receiving standard reply '
'signature mismatch and given various calls to your Helpcenter.

But no solution.

Please suggest the way to resolve this issue.
