Tata AIA Life — Non receipt of premium receipt policy no C[protected]

I had deposited annual premium of my policy no C[protected] vide ch no 000004 of HDFC Bank Sector 11, Panchkula for Rs 3424/-. The cheque was debited to my account on 13.11.2015. Despite my repeated request the insurance company neither sent me receipt of premium deposited nor sent acknowledged my request. Pl. help.
