I want to re activate my prepaid sim immediately. the sim deactivated about 6 months back. what is the procedure and what documents i need to submit. only ID and address proof is enough?

Service request [protected] was generated on 02.06.17 for same.However without resolution, call was closed on 07.06.17.email sent, 10 times called on customer care...No positive result..Really frustrated...
