My pH's I'd 23879856 heaith insu.through paramount TPA I am employee (retired) of SBI submitted three domicillary bills out of three one is passed and two are hold for reason 1) Dr digonesis required &, anathot for date wise sitting in physiotherapy treatment .These two bills are part and parcel of old bill which is sanction by your comp.these bills are with you in my files by marking with pH's I'd .in that passed every thing is mentioned .so no need to submitted srprate report
