Vijaya Diagnostic Centre — Delaying of cheque and improper behaviour

We Lalitha Clinic, Yapral, Secunderabad, Telangana, have received a cheque from Vijaya Diagnostics as a referral amount. But as of that surname was diffent as per pan card and bank details, we have been resubmitted to your marketing person Mr. Anjaneyulu in the month of May and till today we didn't get any information from him. When I called him today 25.10.23, he is asking a self-declaration letter for what the reason for change is of name, I really feel embarrassed is this is the way? after 5 months of returning the cheque, you are asking for clarification.
