Dear Sir,

This is to inform you that I sujeet kr thakur residing in rajiv road santoshi nagar neae khatushyam mandir siliguri Dist DARJEELING west bangal pin no 734005 have applied for voter card for six times but have not get a single chance to go and give vote why the officers are not interisted to issue the vote card, am I have no right to cast my vote what is the rison I want to know what is the fact .

I, Mr. Pranab Kumar Dey, am the resident of the premises at the address stated above since March, 1991, but unfortunately in the last parliamentary elections held in 2009, the serial number against my name had been stamped as deleted from the Voter's list due to reasons unknown to my knowledge.I certify to own a valid Voter's ID card(WB/22/158/0510 58) .Previously on 01.02.2010, I have submitted the form 6 together with various documents(stated below), photographs and a written complaint letter at C.M.C(Calcutta Municipal Corporation)Borough ||.Since, My name is not included in the Voter's list, I could not vote in the C.M.C Elections.

Therefore, I request you to look into the matter such that my name is restored into the Voter's list and please ensure that i can vote in the assembly elections on 27.04.11.The documents that i submitted last time are as follows:-
1.Xerox copy of voter identity card.
2.Xerox copy of passport.
3.Xerox copy of Electric Bill.
4.Xerox copy of Telephone Bill.
5.Xerox copy of Property Tax Bill.

I had submitted my on line filled up form 6 alongwith necessary documents by speed post on 23.03.2011 and the samee has been confirmed received at AERO -31, Vikaspuri by Postal department. However when i tried to confiirm on line about my application. There is a message that your application has not been received at centre please deposit or sent by post your application. I am sorry to say that after passing of one months time I do not know whether it has been received or not.

"Form-6" is for adding your name to the electoral roll. So that your name is included in the voters list and you can vote in the elections.

For getting voter ID, you have to submit "Form-001".

All formalities for voter ID is done by me to representative of election commisson and taking or query about us completed by him since a year but yet not recive Voter ID card .
