(1).I confirms the same address recorded by you for future communication.
(2).The said information which I received through e-mail the amount of Annuity, payable to me is not been mentioned, so kindly arrange to inform.
(3).I shall be thankful to you, if you kindly arrange to inform me the total accumulated amount on the basis of which the amount of annuity is been calculated by your good office.
(4).Kindly confirm the amount of annuity shall be payable to me on yearly basis. In this connection let me know whether I can exercise the option of receiving annuity on half yearly or quarterly basis.
(5).That, as for my service condition the undersigned is eligible for pension as well as superannuation scheme. I hope decision has been taken on one item, and not under both the scheme, kindly arrange to inform me the exact position on this point also.
(6).Your good office shall appreciate that I could not use the said amount which was payable to me for more than two years .And, your being a good big financial organization at international level, shall pay me some interest on this amount, which I shall be receiving shortly as mentioned by you.
I shall be highly obliged for your early reply on all above points.

We have yet not received pension cheque (s) of my mother (Sarla Rani - annuity no. 011M[protected]) w.e.f. June 2010.
I have talked on phone in your office they said that payment stop by Orieintal Ins. due to non receipt of Life certifcate. But I have checked with Orieintal Ins. Jalandhar they confirm that have sent Life certificate in January 2010.
Orieintal Ins. Jalandhar have sent again Life certificate on 17th of July 2010.
Kindly look into the matter and do the needful at the earliest
