Axis Bank — credit card add on card not activated

Address: Karimnagar, Andhra Pradesh, 505001

Dear sir/madam,

I am received my add on card three months back but till now it is not activated. I am continues sending message to particular format but it is not activated, when i use this card, message displayed "your card is declined"
Solve this problem
My add on credit card last 4 digits are: 2762
Name on add on credit card: srujana devi s
Primary card holder name: s ashok kumar
