[Resolved] Axis Bank — Credit Card Duplicate Transactions and Fraud transactions showing in Statement

I have done single Transaction(15, 806) through credit card at Varun motors Pvt ltd at hyderabad on 03 December 2016, But in Current credit card statement showing Two times transactions done same amount .
And Statement generated Transaction showing wrong date formats way
When I Call to customer care they are asking me to submit dispute form and saying to fix issue 4 months of time . From Merchant(Varun Motors Pvt Ltd) side there is no issue but from bank side there is Major technical issue a
