[Resolved] Axis Bank — Debit card application

I have submitted application for debit card 24 may 2017, but not received any debit card till today. Even they do not submitted it after filled by me application form.

I wanted to add this account in upi but i do not have debit card.
And i can not go to home branch again and again because i am at job in gurgaon, udyog vihar, ph-5 122016
