Axis Bank — Refund of wrongly issued debit card I.e: 'debit card issue charges' not received.

I recently opened one NRE Savings A/c in the Axis bank - Telibagh, Lucknow branch. I was issued two debits cards from the bank even when there wasn't any need for the issue of the second card. After clarification with my relationship manager she had admitted that the other card was wrongly issued to me by bank's mistake. My account has been debited for the 'debit card issue charges' for both the cards even though I did not request for the second card as it was wrongly issued.

I have blocked the new debit card as asked to me by my relationship manager. I have personally visited the branch (Telibagh, Lucknow) twice and submit the written application for the same. The response from the branch is not postive. My application is pending with the branch for almost two months as of now and no action is being taken.
