I Brajesh Kushwaha was employed in your esteem organization from-30th Jan 2009 to 28th June 2010 at Nerul branch under the team of Kalpesh Raut (Branch Head).But my salary account has been freeze stating reason not served the given notice period. But the fact is i had submitted my resignation on 28th June 2010.And was ready to serve the notice period. But as per Mr.Kalpesh’s instruction i changed the date and wrote 1st June 10, Because he instructed me that as you are on 28th day of the month if you write 1st of the month it will be consider as serving the notice period and you will get the salary for the month. And after that my salary has been credited to my account but I am not being able to withdraw on account of account freeze. The BM has freeze my account stating reason you have not serve the notice. Where as I did the changes as per his instruction and changed the date on my resignation letter with my sign initial which has been receive by Mr.kalpesh Raut with his sign and stamp.Despite of constant request i was not getting any response from BM.So eventually it got delayed .

Therefore I request you to do the needful to unfreeze my account. Also requesting to send my relieving letter to my communication address. (Given below) & Obliged.
