I, Sudeep Kumar Bose, would like to cancel my Group Health Insurance policy within the Free Look up Period with immediate effect.
My health insurance membership number is 6706575
I had recently purchased a house hold good from Bajaj Finance and despite me not wishing to open a Health Insurance, I found that a Health Insurance has been created against my Loan Account Number - 410LSMEA554086.
I have received the loan document on 9\March 2019 . Request you to kindly cancel this insurance immediately and let me know the needful.
