2008 year Policy Not received till date

Hi
This is Vasudeva, took ICICI Lombard House Insurance in 2007 Dec. But I did not received any policy till now and when I went to ICICI Lombard office at Hyderabad, I found that the address was wrong ( added Taipei, Nepal at the end of the address). I gave written letter to update this and requested to send the original policy to my home address.
Still I did not received it.
Need your help in...

Complaint against ICICI LOMBARD - HOME INSURANCE

Purchase flat on year of 2006 and taken ICICI Lombard home insurance on 2007, suddenly crack has appeared on wall so raised complain no. 60210207 on dtd. 06/02/10 and finally received repudiated letter, when I have talk with local icicilombard office they told me crack happened due to manufacture defects so, your claim repudiated, in my line of thinking if it was due to manufacturing defect then how does it takes four years to appear crack....

NON RECEIPT OF CREDIT-instrument 7300
Sir,
Refer my policy no 4065/0000001/icici/HL/003083 cancelled on 20 JAN 2009 as per request
no.[protected].
As per sms received on 30 jan 2009, refund of Rs 32901/- has been processed and issued thru the
above mentioned instrument 7300.
However I have not received the same till date.
PLEASE HELP.
I request you to, if possible, transfer...
