[Resolved] Axis Bank ATM — Notes get CUT by ATM Machine

Dear Sir
I withdraw the cash from the ATM ID: STCC8664 time: 07:58am DT:03-Jun-12 Record No: 8987 for Rs.15000/- from my Account#[protected], one of the Rs.500 note which has come is cut by ATM and come outside to me.
The 500 note Serial Number:8LU 525763, request you to kindly guide me to whom should i contact to get the replacement Rs.500.

Please find the scanned copy of the Note which i received from ATM, the Phone Banking service executive says i need to contact the AXIS Branch phone number mentioned in the ATM but nothing is mentioned in ATM room.

AXIS Bank ATM — ACCIDENT AT AXIS BANK ATM, KARKHANA

I have gone to the AXIS BANK ATM located at Kharkhana, Secunderabad om[protected] to draw Cash. But on entering the ATM centre while closing the glass door, the glass door suddenly crashed over me, causing very grievous injuries on me and the security in-charge. When i approached the nodal manager , Begumpet office, they out rightly rejected my plea and said the security agency should compensate for the injury caused to me. I have already spent more than Rs. 12000/- for my medical expenses, but so far i have not received any compensation.
