[Resolved]  ICICI Lombard I - Health Care — Mediclaim not reimbursed

We had taken a mediclaim for the full family & when my husband was hospitalized, we were denied the money. I have attached the letter send to them by my husband. Now, when the claim is due for renewal, they have started calling us for the same. We have been paying them for 2 consecutive years.

Dear Sir,
I was hospitalized on 17th May for abscess in the scrotum and was hospitalized in Hinduja Hospital, Mumbai.
I am enclosing all the documents required by you to clear my mediclaim.
1. Letter from Dr. Avinash Katara, Hinduja Hospital addressed to you. (ICICI Lombard).
2. Inpatient Final Bill (Detail) – 3 pages
3. Service Voucher No.ORE182311/10 dt. 17/5/2010 for Rs.900/-.
4. Chemist Bill no.11391 amounting to Rs.1396.20
5. Citibank ATM receipt.
6. Outpatient Receipt/Debit Note 33510 dated 19/5/10 for Rs.360/-
7. Service Voucher No.ORE187893/10 dt. 21/5/2010 for Rs.550/-.
8. In Patient Settlement Voucher for Rs.31,788.00
9. Inpatient Final Bill (Summary) – 2 pages.
10. Information regarding Pending Reports – 2 pages.
11. Histopathology Report.
12. Micro TB Culture Report.
13. Microbiology Report.
14. Culture Anaerobic Report.
15. Hematology Report.
16. Biochemistry Report.
17. ECG Graph.
18. Discharge Summary / Card.
19. Xerox copy of ICICI Lombard Health Care Card.
20. Xerox copy of ICICI Bank Statement dated 18/3/2010.
The above mentioned policy was renewed on 15/2/2010 and the verification was done on the same date by Mr. Dinesh Gharate and Mr. Akash Kulkarni. When I received my bank statement, which I have enclosed for your reference, I called your office to enquire about the EMI deductions, etc. They informed me that the process was on and I need not worry.
When I was hospitalized on 17/3/2010 at Hinduja Hospital, my wife called your office to enquire about the claim procedure, she was shocked to hear that there was no policy taken on our names. After a lot of phone calls and correspondence through internet, they told us that there was a mistake on their part and would rectify the same. They then send us the policy no. and details through courier and internet, but the date of commencement of policy was 18/5/2010.
I have yet to receive my other renewal policy taken for my children Dhiren and Dipti.
Please process my claim and oblige.
