This is to state that by mistake and a mere oversight I had made a online payment of
Rs.25,000.00 (Rs. Twenty five thousand only) against premium of policy no.
[protected] in the name of Mr Vikas Jain on 13/02/2010 via receipt no.
PR[protected] (copy of the same is attached for your reference). I
had received online payment advise through your LIC payment portal a few
days ago. I realized my mistake only a few days later when I checked the
premium calendar online and found that this policy has been wrongly got
registered in my name while registering the same online by typing error.
Generally if you happen to type a wrong policy name or number it doesn’t get
registered at all and the website gives an error while registering. I am therefore myself
amazed at this error and how a wrong person's policy got registered in my name which made me make a wrong premium payment.
Now I would request you to either have the premium refunded to me from
the party or cancel the receipt of the same and credit the amount back into
my bank.
For this I applied to The Branch Manager
