[Resolved]  Mafoi Consultancy — PF form lost & thus no amount transferred to my a/c

The below mail is already drafted to the Manager of Mafoi on 25th Nov 09 & i m awaiting for there revert on the same.

To Manager HR,

My name Kutbuddin H Charniwala currently working with TATA teleservices LTD is mailing the below issue to you as not got satisfactory answer from the Ahmedabad office.

Subject :- Issue related to PF amout not received due to PF form lost by the Chennai corporate office.

Details : I worked in mafoi my from Sep 04 to Feb 05 for Bharti Airtel contract at Ahmedabad location, below are the details of my professional belongings with Mafoi during the tenure.

My Employee id :- 42601
Job Tenure :- Sep 04 to Feb 05 (Approx 18 Months)

Issue in detail is mention below in the mail

I filled the PF form along with the below details before 8 months (i:e in March 09)

• Cancelled check of my ongoing saving a/c of HDFC bank
• Bank statement of the saving a/c for last 6 months
• My ID proof Xerox
• Address Proof Xerox
• Salary Proof Xerox

Last month (i:e Oct 09) I intended to inquire about why the amount is yet not credited in my Bank Account wherein I was told by Mr. Muazzam who is taking care of PF management dept at Ahmedabad that he will mail the same details to corporate office & I need to inquire the same updates to him in a week time. I did what was told & called up Mr. Muazzam to get the updates but he was on leave so I waited for more 2 weeks & called him again on 25th Nov 09.

He told me that the form is lost by the corporate office & thus you are required to fill the form once again which is not acceptable & satisfactory for me. I told him that it is not an employee’s mistake that the form is lost. I did all the activity & provided all the facts & proofs of my a/c to Mafoi but even after 8 months of doing the process if the form is lost then it is not a mistake of the employee who already waited for a long time period of 8 months. I already have waited for 8months & if I will fill the form again then I again have to wait for another 3 to 4 months which is not acceptable by me at any means.

I am expecting a positive revert on the mail in a speedy time, the solution I am expecting is that someone should inquire about the old form & process my PF in my Bank a/c asap, or else I need to approach to the Employee Provident Fund Organization (EPFO).
