[Resolved]  Passport Office Bareilly — late receipt of passport

Sir,
I , Jameel Ahmad applied for the passport of my daughter on January 20, 2011 to Bareilly regional office through my Etah district passport office with the following details: :
Name: Aleena Ahmad date of birth 04.03.1998
Father’s Name: Jamil Ahmad Mother's name shamim bano
Application No.: BLY Z080017 /11
On the website of passport office following message is being displayed -
Furnish a Joint Affidavit from parents duly signed by mother & father on Rs.10 stamp paper regarding purpose of visit from Magistrate of Notary Public.
I have sent the above affidavit IN ORIGINAL to Bareilly office through registered post no. RU[protected] IN dated 17.08.2011 but no responce ireceived from passport office . SO ONCE AGAIN i send a photo copy of the said affidavit to bareilly office by registerd post no.EU 82121096 OIN dated 12.10.2011, AFTER THAT I RECEIVED A LETTER FROM BAREILLY PASSPORT OFFICE demanding a declaration from parents on annexure "H". once again i have sent the obove said declaration { with the original letter which they have sent me on demand by office } by registerd post no.EU[protected]IN dated 11.11.2011 . Till date there is no change in the status of the application. I tried to call several times on the Bareilly office phone no but of no use.
Interestingly, I have applied for my own passport along with the passport for my younger daughter with the above passport application on the same date. I have received mine as well as my younger daughter passport 60 days ago. I have submitted all the documents required in the application already.
Kindl help me to find out the status of my daughter’s passport application.
