Postal Life Insurance — loss of pli

I have enrolled in Indian Air Force on 1992. There i have taken PLI for 20 yrs my proposal no 176215 dated 23 jun 92. My policy no is 492/ANM/9 dated jun 1992 That time my policy bearing Service No. 9215774-B which is now 746607-B SGT Vattem Anjaneya Prasad or V.A.Prasad. I had retired from the service on 31 january 2012 after completing my 20 yrs. Unfortunately, on Jun 2012 i lost my PLI maturity bond while travelling to my sisters house. I request your kind suggestion to how to apply for the duplicate copy of my maturity bond. As my PLI maturity date is on 05 Apr 2014. Kindly, guide me how to apply for the duplicate copy of PLI and where to apply. As, of now Iam staying at Hyderabad in my permanent house which address is mention below.
