Sub: My PLI Policy No.TN/1332-U-EA/55, Maturity over on 30.04.2013 -
Last installment paid October 2012 - Refund the maturity amount -
Reminding you again with details- Request - Regarding.

My Policy No.TN/1332-U-EA/55 dated 14.10.86. But my above policy was changed on 01.10.91 to EA/55 for Rs.50,000/- and premium changed to Rs.175/- and maturity date changed to 30.04.2013 and final premium changed to September 2012 in my original policy by you. Changed original covering letter dated 28.5.1992 sent to you.
As per above PLI Policy, maturity over on 30.04.2013. My premium amount paid upto October 2012 but originally last premium to be paid September 2012 only. One month premium extra was paid.
I am an Indian Oil Corporation Ltd., (IOC) Employee and my premium was paid through my salary on monthly basis from starting to till October 2012, Fully paid.
As per your requirement for claim form, I have enclosed a original Premium remitted letter through my salary, from my Indian Oil Corporation Ltd., Regional Office, Chennai upto October 2012.
After maturity date 30.04.2013 was over, myself sent a mail to your PLI, TN Circle, Chennai and the Superintendent of Post Offices (PLI section) Namakkal Division, Namakkal 637 001, regarding how to claim my PLI Amount.

They sent a Form of Claim for maturity values of postal endowment Assurance Policy. I have duly filled the above claim form and enclosed the following documents.
1. Form of Claim for maturity values of Postal Endowment Assurance Policy(duly filled).
2. My Original PLI Policy No.TN/1332-U-EA/55.
3. My Original conversion letter of PLI policy to EA/55 years duly certified by Chief Post Master General(PLI), Tamil Nadu Circle, Madras 600 002(dated 28.5.1992)
4. Certificate of Pay disbursing Officer regarding recovery at Premium from Pay from Starting to till upto October 2012.
5. My salary slip regarding deducting PLI Premium from my salary Rs.175/- last 13 months upto October 2012.
On 18.05.2013 with above 5 enclosures, handed over to your Head Post Master, Thiruchengode Head Office and they sent to your Namakkal District Officer i.e. The Superintendent of Post Offices (PLI Section) Namakkal Division, Namakkal by Registered Post and they received the same on 20.05.2013. Original PD received mail copy also enclosed herewith for your kind reference.
Past 2 months (from 18.05.2013 to 21.07.2013) I have sent so many mails and also phoned to your Namakkal Superintendent of Post office (PLI Section) several times. They said that my case file was unable to trace in PLI, Tamil Nadu Circle Office, Chennai.
I have paid my premium past 27 years without fail. But my maturity amount not able to get in time. I am extremely feel unhappy that your PLI office not respond my claim.
Kindly make it as my complaint regarding not getting my maturity amount past 2 months. Don’t delay further to send my maturity amount and kindly make it at the earliest.
I am enclosing all Xerox copies of my documents sent, for your reference and do the needful.

After Maturity date , I sent my original Policy with Conversion to EA/55 approved letter
With required documents as per your request, sent to Divisional Office, the superintendent
Of Post office Namakkal. Post two weeks before they said my case file not received.
But now they said my case file is available but in computer not update the conversion
To EA/55 details. They said we asked the Chief Postmaster General, PLI, Tamil Nadu Circle,Chennai.
For seeking approval. But no response from the TN Circle, Chennai.

Post three months senting More mails and letters and phone calls. I case not responded by
Your officials. Please send the necessary approval to release my maturity PLI amount early.
